The SEBLOD Form & List Pack (2 plug-ins) works out of the box!! This is rock & roll - just remember however to put the form field UNDER the list field, otherwise it doesn't show up.

Can you please share how you made the Form field work? For the List field, I see a place to enter the List and Search type but for the Form field I do not see such an option. Also, I agree with you that it's hard to figure out what the options section is for. Thanks a lot for any hints.

Please add a couple paragraphs on how to use this field on the product page. It's a paid plugin and one of the more expensive ones in your marketplace. I think it's OK to let the community work in the dark and experiment on their own with the free products but the paid ones definitely deserve some documentation on usage.
Also, the review, which pretobrazza mentions above, still does not show up on the product page.
